Do you have experienced any major miss-use of the Scaled Agile Framework and found that it feels like a traditional waterfall?

I have some mess-up examples:

  • PI Planning is a 10 weeks waterfall plan
  • hierarchy from strategy, to portfolio, to program, to team responsibility
  • IP Sprint for System test and Quality
  • during 10 weeks iterations, run waterfall process with set Q gates
  • Only implement methods, do not align organization and mindset
  • take SAFe as a blue print and do not learn & adapt

What is your best miss-use example?
